A Subsidy Health and Safety inspection was conducted on 01-30-2019 to review supplemental health and safety requirements. Activities, story time, art time, games, music time, personal hygiene, lunch time, and rest time were observed. There were 35 children present, aged 10 weeks-5 years, with 8 staff, and administrative staff. Staff and children files, and other documentation were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.
Standard #: 22VAC40-665-500-F Description: Based on review of documentation and discussions with administrative staff the vendor failed to maintain compliance with this standard as required relating to posting of children's allergies, sensitivities, and dietary requests. Evidence: A child in care/enrolled had a medically diagnosed food allergy and the allergy was not posted as required so that all staff could be aware of this allergy. There were also several children who had special dietary food requests and this information was not posted as required. Plan of Correction: This information will be posted as required, and all staff will be informed of where to locate the posted list for future reference and compliance. The list will be updated as needed moving forward.
Standard #: 22VAC40-665-580-B Description: Based on record review and discussions with administrative staff the vendor failed to maintain compliance with this standard as required relating to the required pre-service training. Evidence: Staff #1, hire date 09/10/2018, had not yet completed the required 10 hour pre-service training as required. The allowed 90 days to complete this training has since expired. Plan of Correction: The staff member will immediately get the training finished and the proof of completion will be placed within the file for future review. All new staff will continue to be required to complete this training within the allowed time-frame; this will be reviewed by administration monthly to ensure compliance.
