An unannounced monitoring inspection was conducted on January 29, 2019 from approximately 10:00 am to 1:20 pm. Eleven staff were present with eighty-one children. Additional staff were on site for administrative duties. Eight children's records and seven staff records were reviewed.
Standard #: 22VAC40-185-160-C Description: Based on review, facility did not ensure 1 of 7 staff records has documentation of a TB test/screening every two years. Evidence: On January 29, 2019, the record for staff #4 has a most recent TB test conducted September 26, 2016 which is more than two years old. Plan of Correction: Staff will have screening/test completed today.
Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 3 of 8 children's records have documentation of required information. Evidence: On January 29, 2019, the records for child #1 (start date November 27, 2017), child #6 (start date December 17, 2018) and child #7 (start date September 4, 2018) did not have documentation of parent's employment phone number. Plan of Correction: Parents were contacted during inspection and information was added to the children's records.
