A Subsidy Health and Safety inspection was conducted on 01/30/19 to review supplemental health and safety requirements. The inspector arrived at 10:00 AM and departed at 2:30 PM. Nineteen children were in care. A sample size of five staff records and five children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-540-A Description: Based upon review of staff records, the facility has not ensured that they have obtained documentation before staff come into contact with children that staff have received a determination to be free of tuberculosis that was completed within the last 30 calendar days of the date of employment. Evidence: 1. According to the record provided, staff 3 was employed on 1/14/19. The tuberculosis screening was dated 1/28/19. Staff 1 confirmed that staff 3 was on duty with the children prior to obtaining the tuberculosis screening. 2. According to the record provided, staff 5 was employed on 1/16/19. The tuberculosis screening was dated 1/31/19. Staff 1 confirmed that staff 5 was on duty with the children prior to obtaining the tuberculosis screening. Plan of Correction: The facility responded with the following: Administrator was unaware of the change in requirements and will implement the requirement with all newly hired employees.
Standard #: 22VAC40-665-650-E Description: Based upon staff interview, the facility has not ensured that they have developed a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The program director stated that he was unaware of the requirement to have a written policy regarding consistency of care by staff. Plan of Correction: The facility responded with the following: The administrator was unaware of the change in requirements and will develop a written policy and procedure describing how each group of children will receive care by a consistent staff or team of staff members.
