The licensing inspector conducted an unannounced monitoring inspection on 01/31/2019 from 9:37am to 5:30pm. The children were observed playing with toys and games, singing songs, and eating lunch. A formal observation was completed with the toddler and preschool classrooms. Lunch consisted of macaroni and cheese, fruit cocktail, green beans, bread and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and the playground was inspected today as well as the van. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Eleven injury reports were reviewed as well as the center?s injury prevention plan, which was updated 09/04/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 01/11/2019 Last shelter-in-place drill: 06/29/2018 Last fire inspection: 11/13/2018 Last health inspection: 06/04/2018 Today, the following child to staff ratios were observed: Infants - 6:2 Toddlers ? 4:1 Twos - 4:1 Preschool ( Threes and Fours) ? 6:1 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-C Description: Based on review, the center did not ensure that one of five staff records contained documentation of a tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: The record of staff #2 (start date: 3/29/16) did not contain documentation of an up to date tuberculosis screening or testing. Plan of Correction: Staff #2 will go tomorrow and retake the test or get the required paper filled out.
Standard #: 22VAC40-185-240-C Description: Based on review, the center did not ensure that three of five staff records contained documentation of attendance of 16 hours of staff development activities that are related to child safety and development and the function of the center. Evidence: 1. The record of staff #1 (start date: 10/20/15) had documentation of 7 annual training hours. 2. The record of staff #2 (start date: 3/29/16) had documentation of 9 annual training hours. 3. The record of staff #5 (start date: 10/12/15) had documentation of 11 annual training hours. Plan of Correction: We will start on completing annual training tonight.
Standard #: 22VAC40-185-370-5 Description: Based on observation, the center did not ensure that staff provide awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs. Evidence: The inspector observed one infant sitting in a high chair and another infant sitting in a saucer for an hour. During that time the infants were not eating or resting. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-500-A Description: Based on observation, the center did not ensure that staff wash their hands with soap and running water before feeding or helping children with feeding. Evidence: The inspector observed two infant teachers feeding infants without washing their hands before feeding. Plan of Correction: Going forward we will enforce the hand washing policy.
Standard #: 22VAC40-185-510-N Description: Based on observation, the center did not ensure that when an authorization for medication expires, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days were disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: The inspector observed three containers of over the counter medication with expired medication authorization forms. Child #4's authorization form expired at the close of business on 12/31/18. Child #1's authorization form expired 10/5/18. The director informed the inspector that the medication was only given during the authorization period and that the center forgot to return the medication to the parent. Plan of Correction: We will return the medication to the parents today.
Standard #: 22VAC40-185-550-H Description: Based on documentation review, the center did not ensure that the required documentation for vehicles such as potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. Evidence: The center did not have documentation of potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.) on the center's van. The van was used during the inspection to pick up children from school. Plan of Correction: I will work on evacuation routes tomorrow morning.
Standard #: 22VAC40-185-560-K Description: Based on observation, the center did not ensure that tables and highchair trays were sanitized before and after each use for feeding. Evidence: The infant high chair trays were not sanitized before and after feeding the infants in care. The toddler table was not sanitized after feeding the toddlers. Plan of Correction: Going forward we will be sanitizing before and after each meal.
