Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:30pm. There were 85 children supervised by 12 direct care staff. The physical plant, programming, medications, and policies were inspected. Children were observed participating in group play, reading books, and doing art. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-185-380-A
Description: Based on observation, the daily schedule is not posted in every room. Evidence: The daily schedule for children's activities, was not posted in the Toddlers room.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-440-E
Description: Based on observation, cribs are not at least 12 inches apart. Evidence: Cribs in the Infant classroom are 6-8 inches apart.

Plan of Correction: I will move the cribs we don't use and spread out the ones we do.

Standard #: 22VAC40-185-500-A
Description: Based on observation, hands are not being washed after diaper changing. Evidence: Staff and children's hands were not washed after a diaper change in the Infant 2 room.

Plan of Correction: I will review hand washing procedures with staff today.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication authorizations, not all authorizations have parental permission. Evidence: Child #1 has two medication authorizations with only a physicans signature, no parent signature.

Plan of Correction: I will have the parents sign it today.

Standard #: 22VAC40-185-570-E
Description: Based on observation, not all bottles in the refrigerator are dated and labeled with child's full name. Evidence: In the refrigerator in the Infants classroom, bottles for one child were labeled with child's name, but did not include the date.

Plan of Correction: Corrected during inspection.
