An unannounced monitoring inspection was conducted today between 8:35am and 10:00am. 28 children were directly supervised by 4 staff members at the beginning of the inspection. Ratios remained in compliance. The physical space, 7 children's records, staff records, evacuation drills, injury reports and required posted documents were reviewed. No medications were on site. Today was a teacher work day. Children were observed working on crafts, puzzles, electronic games, board games and interacting with staff members. Interactions were positive. Standard #: 22VAC40-185-60-A Description: Based on record review, 2 out of 7 children's records did not contain the required information. Evidence: 2 out of 7 files were missing the parent's annual review to ensure all emergency information was up to date. Child #1 was last reviewed/signed 08/27/2017 and Child #2 was last reviewed/signed 09/20/2017. Plan of Correction: We will review all records and get them updated as soon as possible.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the space, the area was not maintained in a safe and operable condition. Evidence: Caps to cover 3 out of 4 bolts, used to secure the toilets to the floor, were missing. Plan of Correction: We will put in a work order today.
