22VAC40-665 STAFF QUALIFICATIONS & TRAINING

Comments:
A Subsidy Health and Safety inspection was conducted on 02/04/2019 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today. There are 13 children present at LI arrival with 2 staff. 2 staff records were reviewed for specific items only. The entire staff files is reviewed at another time at the main office. 2 children's records were reviewed. See violation notice for missing items. Children were observed participating in a variety of activities in the room. The playground was not in use during today's inspection. Emergency escape drills, first aid supplies, emergency supplies are all in compliance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on a review of records, allergy action plans available for review are not signed by a physician. Evidence: Children with documented allergies, do not have a physician's signature on the allergy action form.

Plan of Correction: I will speak with the main office to update the form.
