An unannounced monitoring inspection was conducted with the director beginning at 3:20 PM and ending at 4:30 PM. There were 68 school age children in the care of six staff members. The children were observed finishing snack which consisted of yogurt, granola, and water. The children were also observed playing large group games and doing art projects. Eight children's records were reviewed. Three staff records were reviewed and in compliance. Seven medications were reviewed and in compliance. The emergency evacuation log was reviewed and in compliance. The most recent evacuation drill was documented as 1/17/19. The most recent shelter-in-place drill was documented as 11/30/18. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent fire inspection was dated 2/15/18. The most recent health inspection was dated 6/6/18. Standard #: 22VAC40-185-60-A Description: Based on a review of eight children's records, it was determined that not all the records were complete. Evidence: Child A's record was missing the address for the second emergency contact. Child B's record was missing the name, address, and phone number for a second emergency contact. Plan of Correction: The missing information will be requested from the parent and the record will be updated.
