Comments:
An unannounced inspection was conducted to investigate a complaint related to staff-to-children ratio, supervision, and care, protection, and guidance of the children in care at the center. The investigation was initiated on January 16, 2019 with the director and assistant director beginning at 8:30 AM and ending at 12:30 PM. Staff interviews were also conducted on January 17, 2019 and January 21, 2019. An exit meeting was conducted with the assistant director on February 8, 2019. Documentation and staff records were reviewed. Classroom observations were conducted. It was determined that the complaints regarding staff ratios and care, protection, and guidance were valid. The complaint regarding supervision was unable to be substantiated. Please provide plan of correction by 2/15/2019.

Description: Based on staff interviews, the center failed to ensure the care, protection and guidance of children in the center's care. Evidence: Staff members stated that on at least four occasions, between December 1, 2018 and January 8, 2019, staff member A, who is under 18 years old was left to supervise the Preschool II A/B classroom alone. It was reported that January 7, 2019 was the most recent date that staff member A was left a lone to provide supervision for the children in the Preschool II A/B classroom during nap time.

Plan of Correction: The center will ensure that no staff under the age of 18 will be supervising children alone.

Description: Based on staff interviews, it was determined that the center does not maintain one staff member for every 10 children for children ages three through five years old. Evidence: Staff members stated that since mid December through the beginning of January, there have been at least three incidents that they were out of ratio.

Plan of Correction: A staff development meeting was held February 18, 2019 and retrained on staff to child ratios and company policies regarding supervision. The center will ensure that each classroom is staffed to supervise the number of children in care.
