An unannounced monitoring inspection was conducted on January 25, 2019 from 11:00am until 12:45pm. Upon arrival to the facility, there were 15 children present with 5 staff members. The children were observed participating in age appropriate activities, hand washing, eating and napping. A sample of 5 children's records and 5 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There were violations cited in the areas of physical plant and background checks. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-280-B Description: Based upon observation the provider did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. During observation it was found that a storage room in the facility containing hazardous substances was not made inaccessible to children. 2. The unlocked storage room contained paint cans that were labeled keep out of reach of children. 3. The program director toured the facility with the inspector confirmed that the storage room was unlocked and contained hazardous substances. Plan of Correction: Will make sure storage room is locked.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and interview, the center failed to obtain a fingerprint background check for any staff who will be alone with, in control of, or supervising children prior to employment. Evidence: 1. There was no fingerprint background check on file for staff #1, #2 and #3. a. Staff #1 has a hire date of 02/9/2017 and was observed caring for children in the licensed program during the inspection. b. Staff #2 has a hire date of 09/09/2016 and was observed caring for children in the licensed program during the inspection. c. Staff #3 has a hire date of 01/22/2019 and was observed caring for children in the licensed program during the inspection. 2. The program director confirmed that staff #1, #2 and #3 did not have fingerprint background checks on file. Plan of Correction: Will send staff to get fingerprints.
