Standard #: 22VAC40-185-340-A Description: Based on interviews and records, staff were found not to ensure the care and protection of a child in care during a self-reported incident on 1/17/2019. Evidence: A child received another child's bottle which contained breast milk. On 1/17/19, around noon, Staff # 1 reported that she noticed there were only two baby bottles of breast milk present for Child # 1, who usually has three bottles. The parent was contacted. About two hours later, Staff # 2 found that Child # 2 had too many bottles of breast milk present. Both staff reported that they then realized that Child # 1's breast milk had been given to the wrong child (Child # 2). Staff report that all bottles had been properly labelled. Plan of Correction: The second parent was notified. Staff were retrained on following the program's protocol of steps to follow prior to bottle feeding. The director reported that there has been spot checking as well.
