Comments:
An unannounced Monitoring Inspection was conducted in the center during the afternoon program on January 23, 2019. There were 26 children present in two classrooms upon arrival at approximately 1:45PM. As reported, there are currently 36 children enrolled in the program. There were 5 staff present in the center, 3 staff were assigned to supervise and lead activities for the two groups of children. A sample of 5 children's records was reviewed. Documentation of the annual fire and health inspections was current. Medications were observed in the center. Staff have completed medication administration training. The children were observed during nap, free play in centers, afternoon snack and dismissal at the end of the program day. Staff were engaged with the children. Findings were reviewed with the Lead Teacher at exit. If you have any questions, please call this Licensing Inspector at (540)309-2310. Thank you for your assistance during the inspection. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a sample review of children's records, the center failed to ensure documentation of a completed physical examination was obtained within the defined time frame; before the child's attendance; or within one month after attendance. Evidence: A sample of 5 children's records was reviewed, documentation of the physical examination in the record of child #3 was not completed in the prescribed time frame, the record noted the date of entry as 08/13/18, the physical examination was dated 12/11/18.

Plan of Correction: Staff will monitor children's records to ensure documentation of physical examinations is retained within the required time frames.

Standard #: 22VAC40-185-70-A
Description: Based on interview with staff, the Center failed to ensure the following information was complete for each staff and maintained in the center: the name, address and telephone number of a person to be notified in an emergency. Evidence: Based on staff interview, documentation of the emergency contact information was not available for review (staff 3).

Plan of Correction: The emergency contact information will be completed and retained for each staff in the center.

Standard #: 22VAC40-185-550-M
Description: Based on reviewed documentation, the center failed to maintain a complete written record of children's minor injuries. The documentation did not have the required signatures: staff and parent signature or two staff signatures. Evidence: A sample of minor injury reports was reviewed, two reports dated 11/14/18 contained one staff's signature.

Plan of Correction: Staff will review written incident reports and will follow center procedures to ensure forms are signed by two staff or by staff and a parent of the child.
