Technical Assistance:
? New subsidy requirements became effective on October 17, 2018. ? The facility is to review the new requirements available on the Department's website: www.dss.virginia.gov

Comments:
A Subsidy Health and Safety inspection was conducted on 1/31/2019 to review supplemental health and safety requirements. The inspector arrived at 2:30 PM and departed at 4:20 PM. Twenty-four children were in care. Nap time, afternoon snack and activities were observed. A sample size of five staff records and five children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A
Description: Based upon review of five staff records and staff interview, the facility has not ensured that staff are evaluated by a health professional and are issued documentation that the individual is to be free of communicable tuberculosis (TB) completed within the last 30 calendar days of the date of employment/coming into contact with children. Evidence: 1. According to the record provided, staff 2 was hired on 12/28/18. The TB screening documented in the record is dated 9/30/18. 2. According to the record provided, staff 3 was hired on 11/28/18. The TB screening documented in the record is dated 1/4/19. 3. According to the record provided, staff 4 was hired on 11/28/18. The TB screening documented in the record is dated 9/20/18. 4. According to the record provided, staff 5 was hired on 11/28/18. The TB screening documented in the record is dated 7/23/18. 5. According to the record provided, staff 6 was hired on 1/3/19. The TB screening documented in the record is dated 10/23/18. 6. Staff 1 confirmed that the timing of the TB screenings for staff 2 through 6 was not within the last 30 calendar of the dates of employment.

Plan of Correction: The facility responded with the following: The administrator was unaware of the changes in the timing requirements for staff tuberculosis screenings. In the future, tuberculosis screenings will be obtained within the last 30 calendar days of the date of employment/coming into contact with the children.
