Comments:
An unannounced monitoring inspection was conducted from 8:30am- 10:45am. Upon arrival there were 52 children in care with 13 staff present. A sample of 5 children's records and 2 staff records in addition to 1 medication. Children were observed playing in learning centers and participating in an art project. Drop off was also observed in the morning.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based, on a record review the center did not ensure all required information was kept for each staff person. Evidence: Staff #1, hired 10/03/18 and Staff #2, hired 01/16/19 did not have an emergency contact.

Plan of Correction: The following corrective action was provided by the director during the exit interview: An emergency contact will be added to both files today.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment, inside and outside, were maintained in a safe condition. Evidence: 1. On the playground there was a large composite play structure that had peeling paint on several of the supporting poles. 2. On the toddler playground there was a tricycle missing a pedal and 3 tricycles missing handle bars.

Plan of Correction: The following corrective action was provided by the director during the exit interview: The tricycles were removed during the inspection and work order was placed for the paint during the inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on a record review the center did not ensure all staff had the required background checks. Evidence: Staff #1, hired on 10/01/18,did not have documentation of a central registry check for Virginia

Plan of Correction: The following corrective action was provided by the director during the exit interview: The address of the employee was placed on the check and the center never received the results. During the inspection the director contacted CPS and sent for a new one.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a record review the center did not ensure all staff had the required background checks. Evidence: Staff #1, hired on 10/01/18, indicated living outside of the state of Virginia within the past 5 years. The center did not have documentation of a central registry check for that state.

Plan of Correction: The following corrective action was provided by the director during the exit interview: During the inspection the director contacted CPS and sent for a new one.
