Comments:
An unannounced monitoring inspection was completed on 02/04/2019. Present during the inspection were 27 children, ages 9 months-5 years, in the care of 7 staff. The inspector reviewed 5 children's records and 4 staff records during the inspection. The staff stated that medication is not administered. The children were observed in the following activities: outside play, free choice activity play, story time, during lunch and during nap time. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The licensing inspector discussed the following with the staff: outside requirements for infants, toddler activities, diaper ointment requirements and subsidy. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all elements as required by the standards. Evidence: 1. The record for Child #1 did not contain information related to the child's allergies and intolerance to food, medication or any other substances and actions to take in an emergency situation. 2. The record for Child #3 did not contain the following information: information related to the child's allergies and intolerance to food, medication or any other substances and actions to take in an emergency situation, health information as required by 22VAC40-185-150 and the name of any additional programs or schools that the child is concurrently attending. 3. The record for Child #5 did not contain the name of any additional programs or schools that the child is concurrently attending and the name of previous child day care and schools attended by the child.

Plan of Correction: The staff will review the records upon enrollment and then again quarterly to ensure nothing is missing.

Standard #: 22VAC40-185-520-C
Description: Based on observations and discussion with the staff, the facility failed to ensure that if diaper ointment or cream is used the requirements listed in the standards are met. Evidence: There was an unlabeled red container of Bourdreaux Butt Paste in the plastic storage container designated for diaper ointments/creams.

Plan of Correction: The staff will review the requirements for diaper ointments and all things must be labeled at drop-off.

Standard #: 22VAC40-185-570-C
Description: Based on record review and discussion with the staff, the facility failed to ensure that a record of each child on formula shall contain the brand of formula and the child's feeding schedule. Evidence: 1. There was no record of the brand of formula and the child's feeding schedule for Child #2. Child #2 was determined to be 9 months old and the staff stated that Child #2 is on formula. 2. There was no record of the brand of formula and the child's feeding schedule for Child #3. Child #2 was determined to be 10 months old and the staff stated that Child #3 is on formula.

Plan of Correction: Will review with infant teachers to ensure the form is signed on the first day.

Standard #: 22VAC40-191-60-B
Description: Based on record review and discussion with the staff, the facility failed to ensure that no person shall be employed until the agency has the persons completed sworn statement or affirmation. Evidence: The record for Staff #3 has a documented hire date of 11/08/2018; the date on the sworn statement was 11/14/2018.

Plan of Correction: The staff will make sure the sworn statement is signed on the first day.
