**Please maintain group sizes as required For all new hires: tuberculosis test or screening is required before coming into contact with children. Documentation must have been completed within the last 30 calendar days of the date of employment. CPR and first aid training is required for all staff within 90 days of the date of employment. Please notify your inspector of suspension or termination of services for more than 24 hours as a result of an emergency. Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.
An unannounced Subsidy Health and Safety Inspection was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see separate report for details). Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection. All staff have documentation of the mandatory VA Pre-service Training and are currently certified in first aid and CPR. A renewal class is scheduled for 2/15/2019. There are 8 children with diagnosed food allergies currently enrolled - please obtain a written allergy action plan for each child. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. The last lockdown drill was conducted 4/12/2018.

Standard #: 22VAC40-665-520-B-11 Description: Based on review of four children's records and staff interview, the facility failed to have documentation of a written allergy action plan for each child with a diagnosed food allergy with the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: four children with diagnosed food allergies did not have a written allergy action plan completed by the physician. Plan of Correction: Center staff will request this information from the parent again.
