Comments:
An unannounced renewal inspection was conducted today from 12:00pm-2:00pm. There were 40 children (12 mos-5yrs.) directly supervised by 7 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, singing songs, playing outside, and eating lunch. Lunch served today was white rice, bean sprout soup, chicken nuggets, and cole slaw. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on children's record review, the center failed to maintain complete children's records. Evidence: 1) 2 of 5 children's files reviewed. Child 4 is missing one parent work phone number. 2) Child 5 is missing two parent work phone numbers, and street addresses for two emergency contacts.

Plan of Correction: We will get the information from the parents as soon as possible.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas in the facility were not kept in a safe condition. Evidence: 1) In the Happy Class bathroom, the top cover of the toilet was removed and propped up against the toilet. The inside of the toilet was exposed and children could potentially reach in and play with the mechanisms. The top cover being propped against the toilet seat is a potential safety hazard to children. 2) In the Joy classroom, the bookcase's lining on the edge is peeling off and hanging off the bookcase causing a potentialy safety hazard for children.

Plan of Correction: The toilet cover was replaced immediately. We will fix the other item as soon as possible.

Description: Based on observation, the center did not maintain the minimum requirements for the first aid kit. Evidence: The first aid kit is missing tweezers.

Plan of Correction: We will purchase tweezers.
