Comments:
The licensing inspector conducted an unannounced monitoring inspection Thursday, February 7, 2019 from 10:45am to 1:20pm. This was a monitoring visit to determine the center's continued compliance since being issued a conditional license. The children were observed engaged in a variety of activities during the center's "work cycle". Afterwards, they were observed eating lunch, and resting on cots. Their meals are delivered form a local agency. Due to construction the center's playground is not in use. Five children's records and three employee records were reviewed during this inspection. Medication is not administered. The center's first aid kit and emergency supplies were inspected. Last fire drill: 01/14/2019 Last shelter in place drill: 01/29/2019 Last Health Inspection: 01/04/2019 Last Fire Inspection: 04/30/2018 Today, the following child to staff ratios were observed: Balanced Mixed Age Group-9:2 If you have any questions about this inspection, please contact your licensing inspector, Roshon Casey, at (804) 662-9760.

Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records on 02/07/2019, the center did not ensure that documentation that each child has received the immunizations required by the State Board of Health were obtained before the child can attend the center. EVIDENCE: The record of Child #2, who started at the center on 10/01/2018, did not contain documentation of immunizations.

Plan of Correction: Notified parent today to bring immunizations today.

Standard #: 22VAC40-185-250-C
Description: Based on observation on 02/07/2019 a notice regarding the presence and location of asbestos containing materials and advising that the asbestos inspection report and management plan are available for review shall be posted. EVIDENCE: The asbestos notice was not posted at the center.

Plan of Correction: Corrected on site. Notice posted during the inspection.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of staff records on 02/07/2019, the center did not ensure that staff obtained a fingerprint based national background check in the required time frame. EVIDENCE: The determination of eligibility letter for the fingerprint based background check in the record of Staff #2, whose date of hire was 10/01/2018, is dated 10/10/2018.

Plan of Correction: Going forward, all staff will have all background checks back before they start.
