Comments:
Today an unannounced renewal inspection was conducted between the hours of 2:20pm and 4:45pm. I observed 32 children directly supervised by 2 staff. Children were observed arriving to the program, watching a movie, washing hands, participating in group play and eating snack. Staff were actively engaged with the children, and assisting them with tasks as needed. The physical plant, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. No medications reported to be on-site. The music room and the gym were found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required documents were posted. Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of immunization records for each child enrolled. Evidence: The records of child#6 and child#7 do not contain documentation of immunization records.

Plan of Correction: We will talk to the parents about it.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical examination for each child enrolled. Evidence: The records of child#6 and child#7 do not contain documentation of the children's physical examination by or under the direction of a physician.

Plan of Correction: We will talk to the parents about the missing information.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the program did not maintain complete children' records. Evidence: Child#1's record is missing the parents' place of employment, work telephone numbers, and two of two required emergency contacts. Child#2's record is missing the father's place of employment, his work telephone number, the mother's place of employment and two of two required emergency contacts. Child#3's record is missing both parents' work telephone numbers. Child#4's record is missing the father's place of employment, the mother's place of employment, and her work telephone number. Child#5's record is missing both parents' place of employment.

Plan of Correction: We will have the parents complete the missing information.

Standard #: 22VAC40-185-430-K
Description: Based on observation and interview, the program did not make provisions to ensure that children had individual place for each child's personal belongings. Evidence: During the inspection there were no individual cubbies for the children's backpacks, the children's backpacks were observed laying on the floor in the music room.

Plan of Correction: We will place their bins in the cafeteria.

Standard #: 22VAC40-185-530-A
Description: Based on record review and interview, the center did not have a staff member trained in first aid, cardiopulmonary resuscitation (CPR) on the premises during the center's hours of operation. Evidence: Staff#1 has a CPR/first aid certificate on-site that expired on 5/25/2018. The staff member does not have documentation on-site to verify that a CPR/first aid certification was obtained.

Plan of Correction: I took the CPR/first aid class on Monday, but I have not yet received the documentation.

Standard #: 22VAC40-185-540-E
Description: Based on observation, emergency supplies were incomplete. Evidence: The battery-operated flashlight and the battery-operated radio were not in working condition.

Plan of Correction: We will replace both items by next week.
