Technical Assistance:
During the inspection, the licensing representative discussed with the provider: - playground resilience surfacing and resources [22VAcC40-185-330. B.] American Society for Testing and Materials STandard F1292-99

Comments:
An unannounced monitoring inspection was conducted today from 10:50am-1:20pm. A total of 17 children were present with 3 staff. A sample of 5 children's records, 1 staff record, medications, emergency drills and other documents were reviewed. Children were observed during craft activities, handwashing, lunch and transitions to nap. Playground not inspected due to weather conditions. Violations:
Standard #: 22VAC40-185-50-A
Description: Based on observations, children's information was not treated confidentially. Evidence: Medical information, including allergies, medications and emergency contacts, were posted visibly in a conspicuous place for Child #6.

Plan of Correction: We will make sure the information is not posted visibly.

Standard #: 22VAC40-185-60-A
Description: Based on records reviewed, a complete record was not maintained for each child enrolled. Evidence: 1. There was no record available during the inspection for Child #3. 2. The record for Child #4 was missing the address for 1 of 2 required emergency contacts.

Plan of Correction: I will make sure to obtain the information from the parents.

Standard #: 22VAC40-185-280-B
Description: Based on observations, hazardous substances were not kept in a locked place that prevents access by children. Evidence: 1. Disinfectant spray was observed on a lower counter - accessible to children - in the kitchen. 2. Several cleansers were observed in a cabinet underneath a sink in the kitchen. A lock was not installed on the cabinet. 3. 2 bottles of hand sanitizer were observed on a low shelf in the pre-k classroom - accessible to children. 4. Several cleansers were observed underneath the sink in the children's bathroom. A lock was not installed on the cabinet. 5. Disinfectant spray was observed on a sink in the children's bathroom.

Plan of Correction: We are going through a full renovation and will install new locks on all cabinets.

Standard #: 22VAC40-185-290-3
Description: Based on observations, protective covers were not installed on all unused electrical outlets. Evidence: 2 outlets were not covered with protective covers in the kitchen where children's meals are served.

Plan of Correction: We will install protective covers.

Standard #: 22VAC40-185-350-E-2
Description: Based on observations, ratios of one staff member for every five children were not maintained in a classroom for children 16 months to two years old. Evidence: One staff member was observed with eight children for approximately 20 minutes in a classroom where the youngest child was 21 months old [Child #4].

Plan of Correction: The staff and I will discuss and plan transitions accordingly to remain in ratio.

Standard #: 22VAC40-185-550-D
Description: Based on discussions with staff, two shelter-in-place drills were not practiced annually. Evidence: There was no documentation of a second shelter-in-place drill during the 2018 calendar year. Staff #1 stated that they were unaware of the requirement of a second shelter-in-place drill.

Plan of Correction: We will make sure to practice two shelter in place drills this year.
