Comments:
An unannounced renewal inspection was conducted beginning at 10:45 and ending at 12:45. There were 25 children in the care of 6 staff. Children were ages 4 months - 4 years. Children were observed having lunch, getting ready for nap, and at circle time. All classrooms were in ratio. Reviewed3 staff files - complete Reviewed 6 children's files - see violation notice Emergency drills including shelter in place are up to date. There are no children on medication at this time. Standard #: 22VAC40-185-60-A
Description: Based on a review of the records for 6 children, it was determined that 1 file was not complete. Evidence: The record for Child #4 did not have a physical exam. Child was enrolled 8/23/18

Plan of Correction: The Director will notify the parents today.

Standard #: 22VAC40-185-550-M
Description: Based on a review of one months accident reports, it was determined that not all forms were complete. Evidence: Several accident reports did not have the time that parents were notified of the accident.

Plan of Correction: The Director will retrain all staff to fill out the forms completely,
