Comments:
Licensing inspector arrived at 8:15am and departed at 11:30am to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of arrival was fifty- three children in care with 9 staff . Additional staff arrived during the inspection. The Licensing Inspector observed children arriving , reading books, playing with blocks, preparing for breakfast, diapering, story time, and games. The sample size consisted of 5 children records and 7 staff records. Areas of non-compliance were reviewed with the assistant director.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on February 7, 2019, the record for child #1 did not have two emergency contact's complete address including city, state and zip code. Staff #7 was unable to locate child #2 ( start date November 26, 2018) record.

Plan of Correction: The missing file for child #2was found the same day , slid into another child's file. Child's one file was corrected the same day as the visit. All children moving forward will have a mandatory paperwork appointment with management before enrollment to ensure compliance.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable. Evidence: During the inspection conducted on February 7, 2019, the following was observed : 1- On the outside of the building the paint is peeling. 2- On both playground composites the paint is peeling.

Plan of Correction: We are working in conjunction with our facilities team to get these two areas repainted on or before March 18, 2019 ( weather dependent). Management will ensure that daily safety checks are noted as being out of compliance when these conditions apply.

Standard #: 22VAC40-185-320-B
Description: Based on observation , the licensee did not ensure the rest rooms were equipped with toilet paper within reach of children. Evidence: During the inspection conducted on February 07, 2019, in two year old bathroom , 1 of 4 toilets did not have toilet paper reachable for the children.

Plan of Correction: Opening staff will ensure that al toilets are checked for supplies at the beginning of each day to ensure proper toilet paper is accessible . This will be checked again midday by each lead teacher. Management will do random checks to ensure compliance.
