22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
An unannounced Renewal Inspection was conducted on February 12, 2019 from approximately 9:50 am until 1:00 pm. At the time of entrance there were 9 children and 6 staff members were present with children. There were additional staff on site for administrative purposes. The sample size consisted of 5 children's records and 3 staff records. Three medications were reviewed. The playground was not inspected due to inclement wether. Children were observed singing, dancing and freely playing with dolls, books and blocks. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the Center Coordinator. Please the licensing inspector, Christine Mahan (757) 404-0568 with any questions.
Standard #: 22VAC40-185-550-F Description: Based on observation and staff interviews, the licensee did not ensure a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone. Evidence: During the inspection on February 12, 2019 in room #12 and #13 the required emergency numbers were not posted by the telephones. Plan of Correction: The coordinator will post them today.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review and staff interviews, the licensee did not ensure 1 of 3 staff records has a Office of Background Investigations (OBI) letter stating eligibility for hire in response to the national fingerprint check prior to beginning employment.
Evidence: During the inspection on February 12, 2019, the record for staff #3 (start date 10-08-18) has a OBI letter of eligibility for hire dated 10-17-18.
Plan of Correction: The coordinator will communicate the information to HR regarding background checks and orientation.
