The licensing inspector completed an inspection in response to a self-reported concern regarding forbidden action that allegedly occurred on 11/26/2018. The Department was notified of the incident on 01/03/2019. On 02/01/2019, the licensing inspector initiated the investigation by conducting phone interviews with witnesses. The licensing inspector was on site on 02/07/19 from approximately 3:00 PM until 3:40 PM interviewing staff, conducting observations at the center and reviewing documentation. On 02/13/2019, the licensing inspector conducted a phone interview with an administrator. The preponderance of the evidence gathered during the investigation does not support areas of noncompliance with the licensing standards related to forbidden action. two staff records and one child's record were reviewed.
