Children were observed engaged in group activities including playing with blocks, sorting, learning about colors and shape, playing outside and using musical instruments. 4 staff and 4 children records, medications and emergency records and supplies were reviewed. Areas of non-compliance are identified in the violation notice. Please contact me if you have any questions at mahrukh.aziz@dss.virginia.gov or 571-835-4718.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were not complete. Evidence: Child # 1's record indicated the first day of enrollment as 2/26/15. Child # 1's record did not have any documentation to show that a proof of of identification was viewed by the center within 7 days of enrollment.

Plan of Correction: I think I have viewed the proof of ID when she started. I have contacted the parent to bring it in for my review.
