Obtaining child protective services registry searches from other states in which an applicant has resided in the past five days was discussed with the program director. Information regarding the registries in other states can be found on the Department's website: www.dss.virginia.gov

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 2:30 PM and departed at 4:20 PM. Twenty-four children were in care with five staff. Transitions from nap time to snack time to activities were observed. It was reported that no medication is administered to children in care. No children's major or minor injuries were reported. A sample size of five staff records and five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-70-A Description: Based upon review of five staff records and staff interview, the facility has not ensure that staff records include all required documentation. Evidence: 1. The record provided for staff 2 did not include the job title and documentation of the required orientation training. 2. Staff 1 stated that staff 2 is serving in the position of program leader. The record did not include sufficient written information to demonstrate that the individual possesses the experience required by the job position. 3. The record provided for staff 3 did not include the job title and documentation of the required orientation training. 4. Staff 1 stated that staff 5 is serving in the position of program leader. The record did not include sufficient written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position. 5. The record provided for staff 6 did not include the job title and documentation of the required orientation training. Plan of Correction: The facility responded with the following: All staff records will be reviewed to ensure that all required documentation is included and available in each staff person's record. Job titles and documentation of qualifications will be clearly verified and indicated.
