An unannounced monitoring inspection took place today from 11:30 am - 1:30 pm. There were 61 children present in four classrooms with 10 staff. I observed children working in learning centers with self-initiated Montessori activities, circle time, reading books, singing and beginning lunch. Records were well-organized. The playground was not inspected today due to weather. If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Standard #: 22VAC40-185-270-A Description: Based on observation today, there were some equipment in the center not maintained in a safe condition. Evidence: A pedal on a tricycle was observed to be broken; there were two holes in the wall at child height in the naprooms, and cords were accessible in the gym. Plan of Correction: The tricycle pedal will be removed, the holes will be patched and the cords will be made inaccessible.
Standard #: 22VAC40-185-510-A Description: Based on a review of records today, a medication authorization was not available for one child of five medication files reviewed. Evidence: The documented medication form for Child # 5 was not an authorization but an allergy action plan signed only by the physician. Plan of Correction: The parent was informed to submit the form as soon as possible.
