Comments:
An unannounced renewal inspection was conducted on 02/11/2019 from 10:37am - 1:30pm. At the time of the tour there were 57 children in care with 11 staff. The children ranged in age from infant - five years. Children were observed during morning program time, lunch and nap. Transportation from public school was observed. The sample size included records for 11 children and five staff. The violation is listed on the violation notice and was reviewed with the program director at the conclusion of the inspection.

Description: Based on observation and interview, the center failed to post a copy of the notice of intent in a prominent place at each public entrance of the facility to advise consumers of serious or persistent violations. Evidence: 1. The notice of intent issued on 01/03/2019 was not posted in the center when the inspector arrived at 10:37am. 2. The program director confirmed that the notice of intent was not posted.

Plan of Correction: NOI was reposted on date of visit 2/11/2019 by the public entrance. NOI will remain in public entrance for 12 months.
