An unannounced renewal inspection was completed today. The physical plant, annual fire inspection, 6 staff files, 5 children's files, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected between 12:20- 2:40pm. The resilient surfacing was unable to be measured due to the weather conditions. There were 49 children under the supervision of 11 staff. During the inspection, children were observed playing outside, working in centers where they counted beads, colored pictures and practiced writing letters and words. They were also observed cleaning up for and resting during naptime. The children bring in their lunches from home each day they are in attendance. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Description: Based on file review, 1 out of 5 children's records does not have all required information. Evidence 1. Child #1 does not have the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: We will ask the parents to complete this.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 6 staff records does not have all required information. Evidence 1. Staff #5 does not have written information to demonstrate that the individual possesses the orientation training required by the job position.

Plan of Correction: An Orientation form will be completed and kept on file.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on file review, 1 out of 6 staff records does not have a search of the central registry (SCR) completed within the past 5 years. Evidence 1. Staff #3's most recent SCR was completed on 10/23/13.

Plan of Correction: We will continue to search for this. If it is not found within the next week, we will have it completed again.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent Violation Notice was not posted. Evidence 1. The most recent inspection was completed on 9/25/18. The Violation Notice (VN) from the inspection on 5/16/18 was posted.

Plan of Correction: These were removed during the inspection. The VN for today's inspection will be posted.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on file review, 1 out of 6 staff does not have documentation of a search of the central registry (SCR) by any other state in which the individual has resided in the preceding five year. Evidence 1. Staff #6 has documentation of having lived in Arizona in the past 5 years. There is no completed SCR for this state.

Plan of Correction: This will be completed.
