63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted on February 11, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 1:30pm and concluded the inspection at 4:30pm. Upon the inspector arrival children were were napping. Upon wakening, children had bathroom breaks and washed their hands in preparation for snack. Later children listened to a book on tape of the "Frog Prince", played a game matching upper and lower case letters and sang songs to finger-plays. Children provide their own meals. All areas of the facility to include classrooms, bathrooms, hallways, and playground area were inspected. Classrooms have age and stage appropriate materials and supplies. The facility had the license, last inspection notice, emergency numbers, emergency evacuation procedures/maps, daily schedule and various parent information posted. The first aid kit and the non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Seven staff records and five children's records were reviewed. Staff on site have current CPR, First Aid and daily health observation training. Staff do not have current MAT training and medications are not being administered. The most recent evacuation drill was on 01/31/2019. The two required shelter-in-place drills for 2018 were completed on 07/03/2018 and 08/08/2018. A lock down drill was conducted on 12/31/2018 and 01/31/2019. Today's Ratio's: Classroom #1: (toddler) 2 children with 1 staff. Classroom #2: (2years) 6 children with 1 staff. Classroom #3: (3 years) 1 children with 1 staff. Classroom #4: (3 years) 4 children with 1 staff. Classroom #5: (4-5 years) 5 children with 1 staff. Fire Inspection: 04/25/2018 Health 05/17/2018 The violations from the licensing period were checked for corrections. There was one repeat violations cited during today's inspection. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the facility did not obtain documentation of a tuberculosis screening at least every two years from the date of the first initial screening for each staff. Evidence: Staff #5 had documentation of a tuberculosis screening dated 02/08/2017, exceeding the two year allowance. Plan of Correction: Per the director: I will have staff make an appointment this week to get an updated tuberculosis screening.
Standard #: 22VAC40-185-210-A Description: Based on review of staff records, the facility did not ensure staff identified as program leaders met program leader qualifications. Evidence: Staff #1 (hired 09/11/2018) identified as a program leader as of 10/05/2018 and observed alone and in charge of a classroom of children on 02/11/2019, did not possess the required education/experience and/or training for the job position of program leader. Plan of Correction: Per the director: I will have staff #1 work with a lead qualified staff until her six months of experience is completed along with her 24 hours of training.
Standard #: 22VAC40-185-240-C Description: Based on review of staff records, the facility did not ensure staff who work directly with children obtained 16 hours of annual staff development that is be related to child safety and development and the function of the center. Evidence: Staff #4, hired 09/06/2016, did not have 16 hours of annual training for the period 09/06/2017 through 09/05/2018. Plan of Correction: Per the director: I missed her year classes. Up to date now. I will keep a chart of year to hire dates to make sure I'm on track with training.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of staff records, the facility did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018. Evidence: The results of the fingerprint-based national criminal background check for staff #3 was dated 10/05/2018 exceeding the requirement to obtain prior to 09/30/2018. Plan of Correction: Per the director: Staff #3 was substituting and had not been recently used is I missed the fingerprint deadline. Everyone is up to date now.
