A supplemental page was provided. The outdoor play areas were measured for square footage.

Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted today, February 19, 2019. The on-site inspection was from 9:00a.m. to 11:50a.m. On this date, there were 16 children present and in the care of two staff members. There were a total of 37 children enrolled in the two morning classes of the preschool program. The classroom was toured and observed. There were five children's records and four staff records reviewed. There were two staff members interviewed, as well as three children. There were no medications being administered on this date which is the policy of the center. Snack was provided by the center and consisted of cheese crackers and apple juice. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The exit interview was conducted on this date. The children seemed to enjoy talking about community helpers. There was an ample supply of toys and equipment available to the children. Some items observed and reviewed included: required posted information which included the license and current inspection findings, daily schedule, emergency evacuation map, updated emergency evacuation plan and injury prevention plan, warm water temperature, first aid kit and emergency supplies, arrival procedures, choice time, opening time, transitions, morning snack, hand washing and bathroom procedures, outdoor time, circle time, story time, and dismissal procedures. Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records and verification from staff, one child's record contained an immunization record which was obtained after the child's first date of attendance. Evidence: 1. The record for Child #2 contained an immunization record that had been signed by the physician stating the child was adequately immunized on 08/10/2017. The first date of attendance for Child #2 was 08/08/2017. 2. Staff A confirmed that the immunization record was received after the child had started preschool.

Plan of Correction: In the future, I will ensure that an immunization record is received before a child's first date of attendance.
