22VAC40-191 Background Checks (22VAC40-191)

An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 7:20 Am and departed at 10:40 AM. Sixty-two children were in care when the initial count was taken. Morning arrivals and transition to morning activities were observed. A sample size of seven staff records and six children's records were reviewed. Injury records and medication administration was reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-240-D-5
Description: Based upon review of records and staff interview, the facility has not ensured that there is always at least one staff member on duty for whom there is documentation of instruction in performing the daily health observation of children. Evidence: Staff 1 verified that the staff records do not include documentation of daily health observation training.

Plan of Correction: The facility responded with the following: Daily health observation training has been included in the first aid/CPR training but it has not been documented separately. Staff will receive the training and it will be documented in the records.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas and equipment, inside and outside, are maintained to be clean and safe. Evidence: 1. There is a metal cover on the toddler playground where erosion has exposed a three inch depth along one corner, creating a potential tripping hazard where a child call fall and be injured on the metal corner. 2. Erosion of the soil has exposed two pieces of concrete aggregate near the egress to the playground from the preschool classroom , creating a potential tripping hazard. 3. There is a two by two inch hole in the wall just above the baseboard behind a toilet in the bathroom between the Dolphin and Starfish classrooms. 4. There are two sections of baseboard peeling away from the wall under the sink in the bathroom between the Dolphin and Starfish classrooms.

Plan of Correction: The facility responded with the following: The lawn maintenance team approached administration recently due to the amount of rain and erosion affecting the playground areas. They will create a lawn maintenance plan addressing these areas. The hole in the bathroom wall will be repaired immediately. The baseboards in the bathroom will be re-glued to be secure to the wall immediately.

Standard #: 22VAC40-185-510-A
Description: Based upon review of medication records, the facility has not ensured that medication is administered according to the center's written medication policy regarding parental authorization. Evidence: 1. There was benadryl and an epipen stored for emergency administration to child 7. The parental authorization on the facility required form expired 1/17/2019. 2. Staff 1 verified that the authorization expired 1/17/2019.

Plan of Correction: The facility responded with the following: The parent completed the form indicating the ten day frame for short term medications instead of indicating the time frame being given for emergency medication. The parent will be required to change the date on the form. This was an oversight on staff 1's part when auditing medication files.
