An unannounced mandated inspection was conducted on 2/19/2019. A walk through of the inside and outside of the program was conducted. The inspector arrived at 9 A.M. and departed the program at 12:30 P.M. There were 87 children present during the inspection. The inspector reviewed nine staff's and children's records. The children were observed during: free play inside and outside, bathroom/handwashing, napping, eating breakfast. Discussion with directors: the three required agreements, new requrements for subsidy.
Standard #: 22VAC40-185-60-A Description: Based on review of nine children's records, the center failed to ensure that all records were not complete. Evidence: 1. Children # 5, and 7 were missing the allergy portion of the enrollment form. 2. Child # 9 was missing chronic physical problems as well as the allergy portion on the enrollment from. Plan of Correction: The director will have the parent to complete the allergy portion of the enrollment form ASAP. All forms will be gone over for completeness.
Standard #: 22VAC40-185-270-A Description: Based on observation of the pre-k classroom, the center failed to ensure that all areas and equipment of the center, inside and outside was maintained in a clean, safe and operable condition. Evidence: 1. The tile floor was missing portions of two squares that was under the desk. It was cracked in several places around the areas. Plan of Correction: The director will work with the corporation to replace the broken tile and will monitor along with staff to ensure that all broken tile is replaced when broken.
Standard #: 22VAC40-185-350-F Description: Based on interviews with director and staff, the center failed to ensure that all children that were assigned to one classrooms were not moved to another classroom repeatedly or to disrupt a child's schedule and attachment to his staff members and group. Evidence: 1. Child # 9 was assigned to 2 year old classroom and was moved to Pooh's classroom repeatedly to meet ratio. Plan of Correction: The director has reassigned the child permanently to the new classroom to ensure that the child is not repeatedly moved or disrupted.
