A Subsidy Health and Safety inspection was conducted on 2/19/2019 to review supplemental health and safety requirements. There were 87 children in care. The inspector arrived at 9 am and departed the program at 12 pm. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements were found.
Standard #: 22VAC40-665-540-A Description: Based on review of nine staff's records, the center failed to ensure that all new staff members had a TB within the required last 30 days of the date of employment or prior to coming into contact with children. Evidence: Staff # 9 was hired on 1/2/2019 and the TB was dated 8/29/2018. Plan of Correction: In the future all TB tests and screenings will be completed as required and the staff person will have a new TB completed.
Standard #: 22VAC40-665-650-E Description: Based on review of policies, the center failed to ensure a policy was written to ensure that each group of children receives care by consistent staff or team of staff members. Plan of Correction: The director stated that this policy takes place, but is not written down. A policy will be written down ASAP.
Standard #: 22VAC40-665-770-B-5 Description: Based on review of procedures, the center failed ensure that the procedures for continuity of operations procedures to ensure that essential functions are maintained during an emergency was not established. Plan of Correction: The director stated that that this is also apart of the process of an emergency, just not a written procedure. A procedure will be in place ASAP.
