Technical Assistance:

The playground could not be inspected today due to the snow covered conditions. The Inspector will email the CDC staff orientation model form.

Comments:

An unannounced monitoring inspection was conducted today. There were 42 children in care, with a total of 9 staff members assigned to their direct supervision. Children's records, staff background checks, staff training, children's medications, emergency preparedness items, the physical plant, and programming were inspected today. This inspection was completed during the middle of the day. Children were observed playing indoor group games and taking their afternoon nap. This inspection was completed between approximately 12:00pm and 2:00pm. Thank you for meeting with me today. Standard #:	22VAC40-185-240-A
Description:	Based on a review of staff training documents, it was determined that one staff person has not received all required components of First Day Orientation training. Evidence: There is no documentation of fully completed Orientation training for staff D.
Plan of Correction:	She worked for me when I operated family day home prior to opening this center. She did the family day home orientation training. I will make sure she completes the Center orientation training.
Standard #:	22VAC40-185-260-A
Description:	Based on a review of administrative documents, it was determined that the center does not have a current annual fire inspection on file. Evidence: The most recently completed fire inspection expired on 9/27/18.
Plan of Correction:	I did not realize we have to contact the fire marshall to schedule the annual inspection. I will call today for the next available inspection.
Standard #:	22VAC40-185-350-A
Description:	Based on observations, it was determined that one staff person was not able to fully provide direct supervision to the children in her immediate care. Evidence: Upon entering the Infant classroom, the Inspector observed staff D using her cell phone while assigned to directly supervising children.
Plan of Correction:	The staff person was using the phone to find music to play during the children's nap time. I will make sure staff members do not use their cell phone at any time while supervising children.
Standard #:	22VAC40-185-450-A
Description:	Based on observations, it was determined that one child was not given a top cover during nap time. Evidence: In the Preschool room, the Inspector observed one 2 year old child asleep on her rest mat without a top cover.
Plan of Correction:	The parent does not want the child to use a blanket. I will ask the parent again.
