Reviewed the requirement for clear documentation of any health problems which may interfere with fulfilling the job responsibilities.

An unannounced Renewal Inspection was conducted on February 20, 2019 from approximately 10:20 am until 2:25 pm. Upon entrance there were 15 children and 2 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 4 staff records. Children were observed during their chapel program. The outdoor play area was not inspected due to inclement weather. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-40-I Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall develop written procedures for injury prevention. Evidence: During the inspection on February 20, 2019, the center did not have an injury prevention plan to review. Plan of Correction: The center director will write a plan and put it in place.
Standard #: 22VAC40-185-60-A Description: Based on documentation review of five children's records and staff interviews, the licensee did not ensure the center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information. Evidence: During the inspection on February 20, 2019, the records for child #1, child #2, child #3 and child #5 did not include the previous childcare/school and the record for child #1, #2, #3 and #4 did not include schools/programs currently attending. Plan of Correction: The center director will write a plan and put it in place.
Standard #: 22VAC40-185-510-E Description: Based on observation and staff interview, the licensee did not ensure medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection on February 20, 2019, two medications were observed Benadryl and Epi-Pen were observed without a child's name and the center director confirmed the medications belonged to children currently in the program , Child #1 and Child #2. Plan of Correction: The center director will have the medication updated with a name and return the Epi-Pen.
Standard #: 22VAC40-185-510-J Description: Based on observation and staff interview, the licensee did not ensure medications, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on February 20, 2019, three medications were observed and they were stored in an unlocked cabinet draw. Plan of Correction: The center director will lock the cabinet.
Standard #: 22VAC40-185-550-D Description: Based on documentation review and staff interview, the licensee did not ensure the center shall implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection on February 20, 2019, there was not documentation of an evacuation drill conducted in September 2018. Plan of Correction: The center director will make sure to document drills in the future.
Standard #: 22VAC40-185-550-M Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. and include all required information. Evidence: During the inspection on February 20, 2019, two of three injury report reviewed did not include the date and time the parents were notified of the injury. Plan of Correction: The center director will talk with staff.
