Technical Assistance:
Use of volunteers discussed. Standard 550.M was discussed regarding documentation of "action to prevent recurrence of injury."

Comments:
An unannounced monitoring inspection was conducted on 02/20/2019 from 10:10am - 12:25pm. At the time of entrance there were 20 children in care with three staff and one volunteer. The children ranged in age from two years - four years. The children were observed during morning program time, lunch and nap. Records were reviewed for seven children and three staff. The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that outside equipment shall be maintained in a safe condition. Evidence: 1. One cozy coup car has areas of cracked and sharp plastic. Also, the lid to the sandbox has areas of cracked plastic. a. Cracked plastic is a pinch point for children's fingers.

Plan of Correction: The broken items will be removed from the play area as soon as possible.

Standard #: 22VAC40-185-550-E
Description: Based on record review and interview, the center failed to maintain written documentation of all emergency practice drills. Evidence: 1. The emergency log contained documentation of one shelter-in-place practice drill for 2018. Two are required. 2. The program director confirmed that the second shelter drill, which was conducted in July of 2018, had not been added to the emergency log for the facility.

Plan of Correction: The second drill was added to the emergency log.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables shall be sanitized before and after each use for feeding. *Sanitized means the surface of the item is sprayed with the disinfectant solution and allowed to air dry after use of the disinfectant solution. Evidence: 1. Staff were observed using disinfectant wipes to sanitize the lunch tables before and after lunch was served to the children. a. Disinfectant wipes do not meet the definition of "sanitized" as defined in the child day center standards.

Plan of Correction: The right sanitizing solution was made during inspection and was used instead of Disinfected wipes. The center will use correct sanitizing solution from now on instead of Disinfected wipes.
