Technical Assistance:
Technical assistance was provided regarding requirements for documenting proof of identification.

Comments:
An unannounced renewal inspection was conducted today from 10:15am-1:00pm. The center had a delayed opening due to inclement weather and children began arriving at 11:20am. There were 48 children directly supervised by 12 staff and all classrooms were in ratio. The physical plant, 5 staff records, 5 children?s records, 3 medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed coloring, building castles, engaging in music class, and receiving individual assistance from teachers with various projects. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-240-D-5
Description: Based on record review and staff interview, there is not at least one staff member who has obtained daily health observation (DHO) training within the last 3 years. Evidence: 1. The center director advised 2 staff members have completed DHO training. 2. Staff #1 last completed DHO training on 2/25/15. 3. Staff #6 last completed DHO training on 2/25/15.

Plan of Correction: We currently have daily health observation training scheduled on March 5, 2019.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, record review, and staff interview, the center's procedures for administering medication do not include methods to prevent use of outdated medication. Evidence: 1. The center's procedures do not address methods to prevent use of outdated medication based on review of the staff and parent handbook as well as interview with the director. 2. In the Quackers room, Child #6's emergency medication was stored in a safety sack. The emergency medication expired in October 2018.

Plan of Correction: I have emailed mom to get the child a new Epi-Pen. I updated the policy and will make a spreadsheet to track medications. I will also check all medications the center currently has and add them to the spreadsheet.
