Technical Assistance:
Licensing inspector discussed annual fire inspection requirements with site director.

Comments:
An unannounced monitoring inspection was conducted today from approximately 1:45pm-4:15pm. There were 24 children in the direct care and supervision of 3 staff. A sample of 5 children?s records, 3 staff records and 1 medication was reviewed. The children were observed during dismissal from school, playing games, coloring, playing basketball, engaged with staff and being dismissed from the program. One new staff member has been transferred to this location since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-185-40-J
Description: Based on observation, injury prevention procedures were not updated annually. Evidence: There was no date on the injury prevention plan. Staff A stated that the form was updated this school year.

Plan of Correction: Staff corrected the plan and added a date during inspection.

Standard #: 22VAC40-185-260-A
Description: Based on observation, the center did not have an annual fire inspection. Evidence: The fire inspection on site was conducted 10/11/2017.

Plan of Correction: The fire marshall will not inspect the school while it is under construction. I will get a written statement from the prinicpal.
