Comments:
An unannounced monitoring inspection was conducted today from approximately 11:30am-3:30pm. There were 60 children in the direct care and supervision of 13 staff. A sample of 8 children?s records, and 5 staff records were reviewed. The children were observed napping, singing, engaged in an art project and being picked up by parents. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 1 of 8 children's records were missing required information. Child A was missing the employer phone numbers of both parents.

Plan of Correction: Parents will update files today.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, staff files were incomplete. Evidence: Staff files were missing required information: 5 of 5 staff files reviewed were missing required information. Staff F gave licensing inspector the job titles of staff as requested. 1) Staff A-E were missing job title. 2) Staff B was missing an address for their emergency contact. 3) Staff A was missing a diploma or transcript to support lead teacher job title.

Plan of Correction: I will update files and the other teacher in the room will act as lead teacher until the other teacher's file is complete.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interviews, ratios were not kept for the youngest child in the group when children were in regularly ongoing mixed age groups. Evidence: While Staff F was on lunch break, which Staff B stated lasted an hour. During this time the "Montessori 2" room was out of ratio. Staff B stated that during that hour there were 23 children ranging from ages 3-5 years old.

Plan of Correction: I will adjust break schedules.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent violation notice was not posted in a conspicuous manner. Evidence: The most recent violation notice was found behind the license turned in horizontally. The violations found during the last inspection were hidden behind the signature page of the summary page.

Plan of Correction: Corrected during inspection.
