Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not ensure that one of five children's records have documentation of an immunization record before the child's attendance. Evidence: Child #5 (enrolled: 11/12/18) had documentation of immunizations dated 11/14/2018.

Plan of Correction: We will make sure they don't enter our center until all records are present.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center did not ensure that the playground equipment's fall zones were free of hazardous obstacles. Fall zones should not include barriers for resilient surfacing. Evidence: The preschool playground had a structure that contained a 4 foot slide with a fall zone measuring 6 feet 7 inches in front of the slide chute. There was a barrier and a fence located at the end of the slide's fall zone. The fall zone for a 4 foot slide should be 4 feet plus the the height of the slide in front of the slide chute.

Plan of Correction: We will put a staff member near the slide when in use.
