Technical Assistance:
An announced inspection was conducted on February 4, 2019 from 12:40pm until 2:15pm in response to request to modify the license. Upon arrival to the facility, there were 12 children present with 4 staff members. The children were observed participating in age appropriate activities, napping, hand washing and diapering. A sample of 5 children's records and 4 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There were violations cited in the areas of the license and administration. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-40-H
Description: Based on record review and interview, the center failed to maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. Evidence: 1. During review of records it was found that the provider did not maintain liability insurance for the physical location of the facility where children were in care. a. There were staff working and children in care during the time of the violation. 2. Staff # 1 identified and confirmed that this location was not covered with required liability insurance.

Plan of Correction: Liability insurance has been requested.

Standard #: 22VAC40-80-120-A-3
Description: Based upon observation and interview the provider did not ensure that the facility operated within the terms of its license, pertaining to the physical location of the facility. Evidence: 1. The license was operating at a physical location that was not identified on the current license for this program. 2. There were twelve children in care at the time of the inspection.
