Comments:
The following Monitoring Inspection took place between the hours of 11:00am-2:45pm. There were a total of 50 children in the direct supervision of 9 staff. A sample of 4 children?s records and 3 staff records were reviewed. The classrooms were organized and there was an abundance of age appropriate materials available. The children were engaged in the following: coloring, building with blocks, identifying shapes, math, learning Arabic, reading stories, and other organized activities. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, each employee did not have an up-to-date TB screening on record. Evidence: 1. Staff #1 had a TB test that was dated 5/21/2016, which expired on 5/21/2018.

Plan of Correction: We sent her right away to get her TB test complete. She returned during the inspection and her file was updated.

Standard #: 22VAC40-185-60-A
Description: Based on record review, each child did not have a complete record on file. Evidence: 1. Child #1 was missing 1 of 2 required addresses of emergency contacts.

Plan of Correction: The parent provided this information during the inspection and the file was updated immediately.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the center did not have documentation of 2 shelter in place drills per year. Evidence: 1. There was no documentation of 2 shelter in place drills on file.

Plan of Correction: We did do the shelter in place drills for the year, but did not document them. We will document them moving forward.
