An unannounced Renewal inspection was conducted in the Center on February 4, 2019. A group of children was observed on the outdoor playground at the time of arrival. As reported, 55 children are currently enrolled in the program. Forty five children were present at the time of the inspection. The children's ages ranged from infancy to five years. The children were separated in 4 groups. The Licensing Inspector observed each group in their classroom. The lunch meal was observed. Staff were observed interacting with the children. The Center was clean and orderly. Each class room was supplied with age appropriate toys and materials which were accessible to the children. Documentation of the annual fire and health inspections was current. A sample of eight children's records was reviewed. Partial staff records were reviewed for 5 staff. Complete staff records are maintained off site and ten records were reviewed at the central office on 01/17/2019. The records were complete and current. Documentation of the practiced emergency drills was reviewed. Medications which were prescribed for children were observed in the center. Findings were reviewed with the Education Coordinator at exit. Standard #: 22VAC40-185-140-A Description: Based on a sample review of children's records, the Center failed to obtain documentation of a physical examination by or under the direction of a physician. Evidence: One of eight records did not have documentation of a PE available for review (child #1). The record noted date of entry as 10/02/18. Plan of Correction: Staff will request documentation of a current physical exam from the child's parent. Records will be reviewed to ensure all information is complete and current.
Standard #: 22VAC40-185-150-B Description: Based on a sample review of children's records, the center failed to ensure each physical examination report and immunization record was signed by a physician, his designee, or an official of a local health department. Evidence: A sample of eight children's records was reviewed, the immunization record for child #1 had not been signed by the physician or his designee. Plan of Correction: Staff will review all physical examinations and immunization records upon enrollment to ensure all have the signature of the physician or his designee.
Standard #: 22VAC40-185-510-A Description: The Center failed to ensure a parental authorization form for the administration of medication to a child was in effect with current dates. Evidence: The parental authorization form for child #2 noted 11/16/18 as the end effective date for an over-the-counter medication. The physician had signed a current authorization form. Plan of Correction: The parent was contacted by the Center and has updated the medication consent form, the form was updated on the same date as the inspection. Staff will monitor medication authorization forms on-going to ensure effective dates are current.
