Comments:
An unannounced renewal inspection was conducted from 7:30 am-8:50am. 43 children were in care with 5 staff members present. Children were observed engaged in a variety of arts and crafts activities, reading books and playing with word puzzles and table top toys. Children were also observed going to the gym with two staff members. 3 staff and 5 children's records, medications. emergency supplies, plan, and emergency drill records were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on records review, the center did not obtain documentation of a physical exam for each child enrolled. Evidence: 1. Child # 1's first day of enrollment was 8/28/17. 2. Child # 2's first day of enrollment was 8/27/18. 3. Documentation of a complete physical exam was not obtained by the center within 30 days of enrollment.

Plan of Correction: I will ask the parents to bring in copies of the physical exam for their records.
