Discussed the following with the director: - documentation of updated physical examinations and immunizations
An unannounced monitoring inspection was initiated today. The physical plant, 6 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 12:15-2:20pm today. There were 83 children under the supervision of 13 staff. During the inspection, children were observed enjoying free play, listening to music, reading books, having a diaper changed (infant), enjoying tummy time (infants), eating lunch and cleaning up from lunch to prepare for quiet time. For lunch, the children were served chicken parmesan, noodles, green beans, oranges and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Standard #: 22VAC40-185-130-B Description: Based on file review, 1 out of 5 children does not have documentation of additional immunizations every six months up to the age of 2 years. Evidence 1. Child #5 is 10 months old. The most recent immunization record on file was completed when the child was 15 weeks old. Plan of Correction: The Director will ask the parents for this.
Standard #: 22VAC40-185-60-A Description: Based on file review, 2 out of 5 children's records do not have the name, address and phone number of 2 emergency contacts in case the parents cannot be reached. Evidence 1. Child #2 only has 1 emergency contact in case the parents cannot be reached on file. 2. Chid #4 does not have addresses for the 2 emergency contacts. Plan of Correction: The Director will ask the parents for this.
Standard #: 22VAC40-185-70-A Description: Based on file review, 1 out of 6 staff records do not have all required information on file. Evidence 1. Staff #2 does not have documentation to verify that the individual possesses the education required by the job position. Plan of Correction: This will be brought in to the center.
Standard #: 22VAC40-185-270-A Description: Based on observations, there were areas of the center which were not kept in a safe condition. Evidence 1. In the Toddler classroom, there was a section where the baseboard molding had been removed from the wall. This exposed peeling drywall. The teachers had placed a small table in front of a majority of this area, but there were still parts that were accessible to the children in care. Plan of Correction: The director requested a work order during the inspection.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on file review, 2 out of 6 staff records do not have completed fingerprint checks. Evidence 1. Staff #3 and #5 do not have documentation of a completed fingerprint check on file. Plan of Correction: The Director stated that these were completed, but unable to be located. She then contacted their auditor to see about getting a copy to keep on site.
