22VAC40-191 Background Checks (22VAC40-191)

An unannounced non-mandated monitoring inspection conducted on February 26, 2019 . There was 35 children and 7 staff present. Additional administrative staff was available to assist. The children were observed freely playing . The facility focus inspection was in standard requirements for physical plant, staff and children records, staff ratio, staff training, fire inspections and background checks.

Standard #: 22VAC40-185-160-A
Description: Based on 1 of 5 records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening within 21 days after employment or volunteering and have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on February 26, 2019, staff#4 (Date of hire (DOH) 11/29/2018) most recent documentation of a tuberculosis screening was 10/05/2015. The tuberculosis screening was more than 12 months old.

Plan of Correction: Staff#4 has a TB appointment schedule for next Wednesday.

Standard #: 22VAC40-185-60-A
Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on February 26, 2019, Child #1's records did not contain a complete address with city state and zip code. Child #3 records did not contain father's work phone number and one emergency contact .

Plan of Correction: Will have child #1's record completed today. Child #3's record was corrected while inspector was here.

Standard #: 22VAC40-185-70-A
Description: Based on 1 of 5 records reviewed and staff interview, the licensee did not ensure each staff record contained all required documentation. Evidence: During the inspection conducted on February 26, 2019, in the record for staff #1 ( hire date 2/11/2019) did not have documentation of two references.

Plan of Correction: Staff #1 will have them completed by the end of the week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure that all areas and equipment of the center inside , was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on February 26, 2019, the licensing inspector observed the following areas of the center were observed: In the school age classroom, on multiple rugs in the classroom there was moveable dirt . Underneath the lockers there was trash, dust and moveable dirt. The hallway to the toddler and school age classroom, the blinds and the window frame had a thick layer of dust. The program director observed the area during the tour with licensing inspectors.

Plan of Correction: Office staff will make sure it is completed.

Standard #: 22VAC40-191-60-B
Description: Based on1 of 5 records reviewed, the licensee did not ensure the staff completed sworn statement or affirmation. Evidence: During the inspection conducted on February 26, 2018, staff #4 ( hire date 11/29/2018) did not have the sworn statement or affirmation form completed before employed.

Plan of Correction: Staff #4 have it completed today.
