An unannounced subsidy health and safety inspection was conducted on 02/27/2019 from 8:55am - 11:25am. A monitoring inspection was also conducted at this facility is a licensed child day center. There were a total of 12 children in care with four staff. The children ranged in age from infant - four years. Children were observed during morning program time and lunch. Nap was observed in the infant class. Records were reviewed for five staff and seven children.
Standard #: 22VAC40-665-650-E Description: Based on policy review, the vendor failed to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The vendor did not have a written policy for consistent care of children according to a review of written policies. Plan of Correction: Plans will be updated by 03/01/2019.
Standard #: 22VAC40-665-770-B-5 Description: Based on policy review, the vendor failed to ensure that the emergency plan for the facility included a written policy for continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: The written emergency plan for the facility did not contain continuity of operations procedures. Plan of Correction: Plans will be updated by 03/01/2019.
