Comments:
The licensing inspector conducted an unannounced monitoring inspection that began at the center at 9:40am and concluded with staff records reviewed at the central office at 1:00pm. There were thirty two preschool children in care: fourteen children with two staff in classroom #1 and eighteen children with two staff in classroom #2. The children were observed listening to a story, playing in the centers, in small group activities and playing on the playground. Four staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) did not contain proof of identity and age. Child's date of enrollment was 08/22/2018.

Plan of Correction: The parents will be contacted today and reminded that documentation of proof of identity and age is needed for the child's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the outside play area in a safe condition. Evidence: 1. The inspector observed the playground. A tree limb has fallen on the plastic fencing surrounding the electrical poles and wires. The fence is broken on one side leaving the area accessible to the children.

Plan of Correction: The fencing will be replaced. The children will not play in this area until the fencing is replaced.
