Comments:
An unannounced monitoring inspection was completed on 2/28/19. There were 17 children present with two staff supervising. Five children's records and three staff records were reviewed. There were no medications. The inspector arrived at the school at 3:45 pm and departed at 5:45 pm. The children were observed during homework time, during outside time, and during departures.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center failed to ensure that each child had documentation that the child had received the immunizations required by the State Board of Health before the child could attend the center. Evidence: 1. There was no immunization record in the record of child #1. According to the staff, child #1 first day was some time prior to winter break (December 2018).

Plan of Correction: The staff will request a copy of the child's health record from the public school.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, there was no documentation of a physical examination for each child within 30 days after attendance if not obtained prior to enrollment. Evidence: 1. There was no documentation of any physical examination report from a physician in the record of child #4. Child #4 had a documented first date of attendance as 8/13/16. 2. There was no physical examination in the record for child #1. According to staff, child #1 was enrolled some time prior to winter break in December 2018.

Plan of Correction: The staff will request the health record from the public school.

Standard #: 22VAC40-185-60-A
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that all required information was documented for each child's record. Evidence: 1. The record for child #2 was missing documentation of the child's first date of attendance. One of two required emergency contact addresses was not complete - there was a street name and city only (no house number). 2. The father for child #6 was listed as not authorized to pick up the child. The name of the person not authorized to pick up the child was verified through the child's birth certificate that this name was the biological father of the child. There was no legal paperwork to support this.

Plan of Correction: The staff will talk with the parents to get this information and document it in each child's record.

Standard #: 22VAC40-185-260-B
Description: Based on observation and interview, the center failed to ensure that there was annual approval from the health department. Evidence: 1. The last "Report of Environmental Sanitation Inspection" form was completed 9/20/17. 2. The staff was asked if there is a report that was completed within the last year and the staff stated that they were not aware of one.

Plan of Correction: The site director will contact the child care director to have a report form completed for this year.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the center failed to ensure that the findings from the most recent inspection ("Violation Notice") were posted at the facility. Evidence: 1. Based on observation, the "Violation Notice" from the August 2018 inspection which had three violations was not posted. The inspection results from the January 2018 inspection were posted.

Plan of Correction: The "Violation Notice" from today's inspection will be posted.
